Dear Teachers, Parents and Local Community,


We are pleased to inform you that the Lake Ridge School-Based Health Clinic (LRSBHC), located in Calumet High School, will extend its medical and behavioral health care services to the local community starting Monday, April 19, 2010.  The community hours of operation will be Monday through Friday from 3 p.m. until 8 p.m.

This opportunity is being made available to the community of Lake Ridge, as an expansion of Lake Ridge Schools (LRS) partnership with Community HealthNet, Inc. (CHN), a Federally Qualified Health Center, who has sponsored (2) School-Based Health Centers (SBHCs), on school grounds, at Calumet High and Grissom Elementary schools.

The community clinic will be staffed by a Family Practice provider and will have five exam rooms.  The clinic will offer primary and preventive health care services, including annual exams, well child check-ups, acute and urgent care, lab services, chronic disease management, sports physicals, as well as, many other services.  These services will be provided regardless of ability to pay.  All insurance plans are welcome and for the uninsured the cost of care is based on a sliding fee scale and family income.

Prescriptions from the health care providers at the Calumet clinic can be conveniently filled at the Fagen Pharmacy located directly across the street from the clinic.  Uninsured patients will receive special discounted rates only offered to Community HealthNet patients.

As we continue our efforts in providing LRS students with medical and behavioral services, we are very excited to move forward in our planning efforts by extending these services and more to the local community.  Walk-ins are welcome; however, if you would like to make an appointment, please call (219) 880-1190.
